4. Health Information

To help us provide safe and appropriate massage services, all clients are required to complete a Health Intake Form before their first appointment and to update their health information whenever there are significant changes.

Please complete your Health Intake Form carefully, honestly, and to the best of your knowledge. Providing accurate and complete information allows us to determine whether massage therapy is appropriate, identify any necessary modifications to your treatment, and provide services as safely as possible.

If your health condition changes after submitting your Health Intake Form or before a scheduled appointment, please inform Evelion as soon as possible so we can determine whether any adjustments to your treatment are needed.

 

5. Client Responsibilities

Clients are encouraged to:

  • Communicate their goals and expectations before the session.
  • Inform their therapist of any discomfort during treatment.
  • Request adjustments to pressure, positioning, temperature, music, or other aspects of the session at any time.
  • Notify their therapist immediately if they wish to pause or stop the massage.
  • Follow any reasonable post-session recommendations provided by their therapist.

Open communication helps us provide the best possible experience.

 

6. Draping and Client Comfort

Client comfort, privacy, and dignity are our priorities.

During the booking process, clients may choose their preferred draping option from those offered by Evelion. Regardless of the option selected, appropriate draping will be maintained throughout the session in accordance with professional standards, client comfort, and applicable laws and regulations.

Your therapist will always respect your comfort level. If you have questions or wish to modify your draping preference before or during the session, please let us know.

 

7. Professional Environment

Evelion is committed to providing massage therapy in a respectful, safe, and professional environment.

Clients are expected to behave respectfully toward their therapist and to follow reasonable instructions that promote a safe and comfortable treatment experience.

We reserve the right to refuse or discontinue services if circumstances arise that make it unsafe, unsuitable, or otherwise not in the best interest of the client or therapist to continue treatment.

 

8. Contraindications

Certain medical conditions may require modifications to your massage or may make massage therapy unsafe or unsuitable until your condition has improved or medical clearance has been obtained.

Depending on your health condition, we may:

  • Modify the treatment plan.
  • Recommend postponing your appointment.
  • Request medical clearance from your healthcare provider.
  • Decline treatment when necessary for safety.

Our primary responsibility is to provide massage services safely and responsibly.

 

9. Informed Consent

By scheduling or receiving massage services from Evelion, you acknowledge that:

  • You have read and understand this Health Information & Informed Consent document.
  • You have had the opportunity to ask questions before receiving services.
  • You understand the general purpose, benefits, and limitations of massage therapy.
  • You have completed your Health Intake Form honestly and accurately to the best of your knowledge.
  • You understand that you may communicate with your therapist and request adjustments or discontinue the session at any time.
